چکیده

A safe, practical, clinically applicable, noninvasive method for measuring local cerebral blood flow (LCBF) using inert xenon (Xes) and a 60 sec CT scanner has been developed in the baboon. Direct measurement of expired Xes concentration after short inhalation (4--7 min) of 40% Xes and the use of computer-programmed autoradiographic formulas allow accurate, reproducible measurements of LCBF using serial 60 sec scans of regions as small as 0.04 cm3. LCBF measurements are possible with a single 1 min scan. The method reduces radiation exposure, obviates a costly fourth-generation scanner, avoids anesthetic effects of Xes, and reduces the 30 min/scan saturation period. It is less expensive than emission tomography and minimizes problems of overlap and Compton scatter inherent in 133Xe and positron-emission blood flow measurements. Regions of zero perfusion are demonstrable in three dimensions. Tissue solubility and partition coefficients, as well as LCBF, are measured in vivo with high resolution and reproductibility so that minor regional changes in physical properties of tissue that alter solubility are measured. These enhance the potential clinical usefulness of CT scanning.

Effects of inhaled stable xenon on cerebral blood flow velocity.

The effects of inhaled stable xenon gas on cerebral blood flow were studied with 23 transcranial Doppler examinations performed in 13 normal volunteers while breathing, 25, 30, or 35% xenon for 5 min. Doppler velocities from the middle cerebral artery rose significantly during inhalation in 85% of subjects and 78% of studies and decreased significantly in 15% of subjects and 17% of studies. The...

The effect of stable xenon on ICP.

Recent studies have suggested that under certain conditions, inhalation of stable xenon can cause an increase in CBF or intracranial pressure (ICP). We reviewed the ICP changes that occurred during 48 stable xenon/CT CBF studies in 23 comatose head-injured patients to determine if the concentration (32%) and duration of inhalation (4.5 min) of stable xenon we used caused an increase in ICP. In ...

Effect of stable xenon on regional cerebral blood flow and the electroencephalogram in normal volunteers.

We evaluated the effects of breathing 35% stable xenon in 65% oxygen on regional cerebral blood flow and the electroencephalogram in 20 normal volunteers. We measured blood flow in 32 brain regions over both hemispheres with the xenon-133 intravenous injection technique in two protocols. In the first protocol (n = 10), a baseline study was followed by a second study during 5 minutes of breathin...

Xenon Inhalation on Whole-Brain Blood Flow and Metabolism in Awake and Fentanyl-Anesthetized Monkeys

Background and Purpose: Despite the documented diagnostic value of local cerebral blood flow maps by xenon-enhanced computed tomography, reports of cerebral blood flow activation by inhaled 33% Xe raised concerns about the method's safety and accuracy. We evaluated the effect of 33% Xe inhalation on cerebral blood flow and cerebral metabolic rates for oxygen and glucose in four awake and six fe...

Accuracy of xenon CT measurement of cerebral blood flow.

In recent years, quantitative determination of cerebral blood flow (CBF) by xenon-enhanced CT has become an increasingly important research and clinical tool. Concurrently, the possibility that inhalation of stable xenon in the concentrations required for this procedure might itself alter CBF also has received increasing attention. Several studies [1-6] have examined this problem by using vario...
